    Payment & Insurance

    As a courtesy for those of you who have Blue Cross, United Health
    Care, Healthlink or Medicare insurance, Dr. Fazio will prepare and
    send all primary coverage submissions.  All visits are submitted daily.
    Please understand that our office is only the liaison between you and
    your insurance company. Holistic Therapy appointments cannot be
    submitted to insurance.

    All clients are expected to pay in full at the time of service, and once
    your out of network deductible is met you will be reimbursed from the
    insurance company directly.

    You may pay by cash, check or for your convenience you may charge
    your balance to Visa, MasterCard or Discover and pay them direct as
    your budget allows.

    All nutritional supplements must be paid in full each visit or by credit
    card if mailed.

    Appointments & Cancellations

    We ask that 24-hour notice be given if you cannot keep your
    scheduled appointment so that we may serve others in need.  
    We realize that mistakes happen, each client will be given 1
    miss per year, additional missed appointments will result in
    payment in full for their missed appointment.

    We appreciate the opportunity to participate in your journey
    to better health and wellness.
